Based on observation and interview, the licensee did not comply with the section cited above in 1 play yard another mattress was added to prevent child from hitting their head on wall causing sheets to be not fitted which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will remove additional mattress and send proof of completion via email.

Based on observation, the licensee did not comply with the section cited above in 1 play yard has a sheet attached to the side and a blanket hanging over the side which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will remove sheet and blanket from side of play yard and send proof of completion via email.

Based on observation, the licensee did not comply with the section cited above in 2 infants were alone in attached room in their play yards with sliding glass door closed while assistant was in another room with sliding glass door closed with 5 children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she create a plan to ensure the children are not alone and submit proof of completion via email to LPA.

Based on observation and interview, the licensee did not comply with the section cited above in assistant was alone with 7 children, 3 infants and 4 preschool aged children while Licensee stepped away from home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she create a plan to ensure that they are in ratio at all times and submit proof of completion via email to LPA.

Based on record review, 3 out of 7 children's records were missing LIC 627 form or any emergency medical consent, therefore the licensee did not comply with the section cited above,which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA with LIC 627 for C3, C4, and C7 via email by 11/15/24

Based on record review, LPA observed 4 out 7 children's records to be missing proof of immunization, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of immunizations for C1, C4, C6 & C7 to LPA via email by 11/15/24

Based on record review, 1 out of 1 infant under age one was missing the LIC 9227, therefore the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of a completed LIC 9227 for C7 via email by 11/15/24

Based on observation, interview, and record review, the licensee was operating in a converted garage space that has not been cleared by the Escondido Fire Department and is not covered by the current fire clearance on file, which does not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will cease using converted garage area for childcare purposes by 10/25/24 and use only the approved "on-Limit" areas until a fire inspection and clearance to use the space has been granted by the Escondidio Fire Department. Licensee will provide proof with photographs that the space is not in use and the areas now being used via email or text to the LPA by 10/28/24

Based on observation, interview, record review, S2 was providing care and did not have a cleared criminal background record, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will S1 re-do fingerprints and submit the receipt to LPA via email by 10/25/24. Licensee understands that S2 is not allowed to be on site or provide care until full clearance is granted. Once cleared, Licensee will provide LPA with letter via email.
