Based on observations the facility had classroom with steps that have exposed screw heads sticking out, cracked plastic red chair and a peeling blue vinyl child’s couch which poses a potential health, safety and/or personal rights risk to children in care.

Based on observations, the facility’s front office and 2’s classroom was cluttered with supplies and equipment underneath the receptionist area and the back of the 2’s classroom that was accessible to children which is a potential health, safety and/or personal rights risk to children in care.
Based on interviews the after-school staff did not realize a child had left the classroom and exited the building and was only aware of it when a parent brought the child back into the facility.
Based on observations, the facility’s kitchen trashcan was overflowing with trash and did not have a tight-fitting lid which poses a potential health, safety and/or personal rights risk to children in care.
Qualifications: (1) Teacher Aide Qualifications and Duties. (e) An aide shall work only under the direct supervision of a teacher. (1)An aide who is 18 years of age or older, and who meets the requirements of this section and Section 101216, may escort and/or assist children in going to the bathroom, and may supervise napping children as specified in Section 101230(c)(1), without being under the direct supervision of a teacher. This requirement was not me as evidenced by: Based on records review of timecards, schedules, sign in and sign out sheets and interviews it was proven that a teacher aide provided supervision of children on several occasions over a period of two weeks during January 13-26, 2021, when the children were awake, without being under the direct supervision of a teacher. There was not enough qualified staff clocked in during January 13-26, 2021, to cover breaks and lunches for the number of classrooms open and the children present, as the substitute director did not always cover the breaks and lunches.

This requirement was not as evidenced by: Based on observations and confidential interviews, the facility has an issue with cockroaches and dead cockroaches were observed under the sinks in the kitchen and the 3’s classroom.

This requirement was not met as evidence by: Based on observation, the facility was observed to have dirty walls, floors and rugs and not in good repair. The floors in the Pre-K, middle room and 3’s room had broken floor moldings and worn/torn carpet. Also, the Pre-K and 2’s classroom had rugs that were dirty, stained and worn which is a potential health, safety and/or personal rights risk to children in care.
