Based on observation and interview, the licensee did not comply with the section cited above in 1/3 persons working with the children in care did not have evidence of a current tuberculosis clearance, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain TB test results for all staff and provide a copy to licensing no later than 07/14/2025.
Based on observation and interview, the licensee did not comply with the section cited above in that the Department was not notified of the on going alterations in the downstairs bathroom that children in care use which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will write a letter to LPA confirming construction will not occur while children are present and will also put a fence to ensure children do not have access to the pending construction in the bathroom.

Resolution: Licensee agrees to complete the andated Reporter training and submit certificate to LPA via email by 10/6/23.
