Resolution: Licensee stated she will obtain assistants vaccines and TB and submit proof of completion via email to LPA.

Based on observation, the licensee did not comply with the section cited above in C4 was swaddled while napping which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the blanket from the C1 napping in play yard. Licensee will provide written statement advising of licensee's understanding of safe sleep regulations.

Based on record review, the licensee did not comply with the section cited above in C4 did not have Individual Infant Sleep Plan LIC 9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall have the LIC 9227 completed by parent and submit proof to LPA via email on or before POC due date. Licensee shall also provide a statement of understanding for the Infant Safe Sleep requirements.

Present at the facility was licensee Maria DeLourdes Ferrer, Assistant Daisy Saldana, licensee’s spouse Nicasio Ferrer and son Ricardo Ferrer. Licensee and assistant were caring for four children. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: · Normal days and hours of operation are: 6:00 AM to 6:00 PM, Monday-Friday · Off-limit areas include: Bedrooms and hall bathroom · The facility is licensed to have no more than 14 children as a large FCCH and is operating within the licensed capacity and appropriate ratios. · Appropriate supervision provided during this inspection · A working telephone is present, and the current phone number is on file · A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection. · All hazardous items are stored inaccessible to children · Toxins are locked under kitchen sink · Weapons are not present.
