Based on record review, the licensee did not comply with the section cited above in 1 out of 2 persons did not have a completed mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: By COB, Licensee agrees to submit proof of current mandated reporter certificate for assistant, to CCL.
Based on observation, the licensee did not comply with the section cited above in that play equipment located outside had a tear in the plastic which causes an opening which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have have the opening on the play equipment fixed and or replaced, and will send proof via email to LPA once it has been fixed. Licensee will make the play equipment off-limits until it has been fixed.
Based on observation, the licensee did not comply with the section cited above in that locks on the bathroom cabinets were not working and children had access to items labeled "keep out of the reach of children" and cleaning solutions left on the kitchen counter which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee had new lock installed in the bathroom cabinet while LPA was onsite. LPA verified lock is now wokring. Licensee also removed and locked cleaning supplies.
