Based on observation, and interview, the Licensee did not comply with the section cited above in that she did not notify the Department of alterations and additions to the property, which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee will provide an updated sketch to the Department by 11/03/2025.

Based on observation, and interview, the Licensee did not comply with the section cited above in that she did not notify the Department of a pool, that does not meet licensing regulations, being put on the property, which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: The Licensee stated that she will notify the Department if she decides to close her facility, or put up a fence that meets licensing regulations by 11/24/2025.

Based on observation, and interview, the Licensee did not notify the Department of a pool, that does not meet licensing regulations, being put on the property, which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: The Licensee stated that she will notify the Department if she decides to close her facility, or purchase the life ring, life pole, and conduct/document pool logs, by 11/24/2025.

Based on observation and interview, the Licensee did not comply with the section cited above in that the LPA observed 20 children at the home, when the license capacity is 14, this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she runs an outdoor school with a current enrollement of 14 children, that she will move off of her property, by 10/24/2025.

Based on record review, the licensee did not comply with the section cited above in that there were no sleep logs in place for the three children under two years. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that they have been checking the children but not logging it. She will establish a log and initiate the process right away. Documentation of next week's logs will be sent to Licensing by 3/31/23 for verification of corrrection.

Based on record review, the licensee did not comply with the section cited above in that she did not have the form on file for the 10 month old infant in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will have the parent completed the form right away and a copy will be sent to Licensing by 3/31/23 for verification of corrrection.
