Resolution: Substiute and/or licensee will submit a criminal record transfer request (LIC 9182) to cclpsacs@dss.ca.gov for Staff 1 by the POC Due Date, 8/24/23 and will send proof of the submission to LPA. Staff 1 will not return to the facility until they are cleared and associated to the facility. Substitue and/or licensee will email proof of Staff 1's association or confirm by calling LPA.
