Based on observation, the licensee did not comply with the section cited above in that the gate does not self latch, which poses an immediate health, safety or personal rights risk to persons in care. Licensee keeps the gate locked with a keyd lock.
Resolution: Licensee will correct the gate so it self closes and latches upon opening. Licensee will email LPA Beckby a video of the corrected gate by due date, or LPA will return for a follow up inspection.
***This is an amended report.***Based on record review, the licensee did not comply with the section cited above in which1 employee submitted fingerprints on June 2024 to the department but was not associated with the facility, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will have employee resubmit fingerprints and wait for clearance from Guardian and/or call Guardian for association.
