Based on LPA observation, the licensee did not comply with the section cited above when an infant was placed in a play yard with blankets which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee Rodriguez will review the Safe Sleep documents which LPA provided and review the safe sleep website (website link on 809 Report). Licensee will sign a statment declaring that she reviewed both the Safe Sleep documents and website and email the signed statement to LPA by 5 PM on 01/30/2024.
Based on LPA observation the licensee did not comply with the section cited above when LPA was not able to review children's files with all required documents including th emergency information which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA will conduct a file review during an unnounced field visit after 02/29/2024.
