Based on observation and interview, the licensee did not comply with the section cited above in which she and her assistant took online classes to satisfy First and CPR, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assisant will enroll in an in-person training and will email LPA a copy of the reciept stating enrollment with date and send CPR & First Aid cards from either American Heart Assosication, Red Cross (in person), or EMSA certified trainer.

Based on interview and record review, the licensee did not comply with the section cited above in 32 out of the 2 infants do not have recorded sleep logs, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to check on sleeping infants and docuement every 15 minutes. LPA will return to verify logs are being kept.

Based on observation and interview, the licensee did not comply with the section cited above in that she had 13 preschool children during the time of inspection, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to disenroll one child. Licensee will submit a statement saying she understands she must maintain licensing ratios for the safety of the children. LPA will return to verrify compliance.

Based on observation, the licensee did not comply with the section cited above in which the fire extinguisher's needle is in the "recharge zone", which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a new fire extinguisher or get it serviced. LPA will return to verify working fire extinguisher is availablle.

Based on observation, the licensee did not comply with the section cited above in which Licensee has 14 children (13 under 5 years old and one school age child) enrolled, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee corrected the deficiency by sending one sick child home. However, the License is still over capacity with enrollments. Licensee will disenroll 1 child to comply. Licensee will call parents and disenroll within 24 hours. LPA will make a return visit to ensure compliance

Based on observation and interview, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA observed the ammunition to not be stored seperately from the firearm. LPA observed the firearm and ammunition to be locked in a safe in an off-limit area out of the reach of children.

Resolution: Licensee will ensure to store and lock ammunition separately from the firearm by the POC due date. Licensee stated she does not operate on weekends, but will complete the POC by Saturday 9/25/21. The firearm and ammunition will be inspected by an LPA by Monday 9/27/21.
