Based on record review, the licensee did not comply with the section cited above. LPA noticed Licensee did not record 15 minute sleep checks since 08/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start documenting sleep checks and will keep the records for review. LPA will conduct a POC visit.
LPA observed that Licensee and her assistant did not have current Pediatric CPR and First Aid certifications. Licensee and her assistant’s certifications both expired in December 2020, which poses a potential risk to the health and safety of children in care.
Resolution: During today's visit Licensee and her assistant provided LPA with confirmation of enrollment in an approved EMSA course on March 3rd and 4th. Upon completion of the course they will provide current copies of their certification to LPA.
LPA observed 6 of 8 children’s files that had incomplete immunization records or no immunization records on file. This poses a potential risk to the health and safety of children in care.
Resolution: Licensee will request updated immunization records from all families in care and provide LPA with the completed documents.
