Based on observation, the licensee did not comply with the section cited above in that upon entry, LPA noticed the gate leading upstairs was not closed, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remind all family members and staff to close the gate when going up and down the stairs. Licensee will make "Close Gate" posters next to gate as a reminder to close the gate.
Based on observation & interview, the licensee did not comply with the section cited above in that she had 15 preschool children at time of inspection, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will disenroll and tell Drop in parents she is full. Licensee will email LPA a list of enrolled and disenrolled students, LPA Beckby will return to verify compliance.

Based on record review, the licensee did not comply with the section cited above in 8 out of 13 children's files did not contain updated immunization records and/or immunization information was not transferred to the required forms, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ask for updated immunizations for all parents and transfer the information to the required forms. LPA Beckby will return to verify compliance
Based on interview & record review, the licensee did not comply with the section cited above in 1 out of 1 of her current employees does not have MMR immunizations or proof of immunity, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Employee will call doctor and get blood work done to see if she has immunity, or get vaccinated. Licensee will email LPA copy of results.

Resolution: Licnesee will have assistants fill out all forms and documents needed to comply. LPA will conduct a visit to verify compliance.

Resolution: Cleaning agents were moved and placed out of reach of children during inpsection. CItation was corrected at time of visit.

Resolution: Son will get fingerprinted and assistant is not allowed back until fingerprints have cleared.
