Based on observation, the licensee did not comply with the section cited above in that the Fire Extinguisher was last serviced on 9/25/23 as indicated on service tag which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have the fire extinguisher serviced and submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in that Poisons were observed not to be locked under key in the front part of the backyard which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During inspection Licensee placed poison in a shed under combination lock. Licensee, will submit decleration on plan to keep poisons under key and how children are supervised when walking to the outdoor yard used for play.

Based on observation, the licensee did not comply with the section cited above in that LPAs observed a kiddy pool with dirty water and a dog water dispenser filled to the rim with water accesible to the children in the back yard used by children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, children do not use the backyard at this time. LPA advised that water should be emptied. Licensee, will review bodies of water regulation and submit written plan on how to remain in compliance by 9/25/2023.

Based on observation,the licensee did not comply with the section cited above in the home to not be oderly- food particles were observed on the floors throughout the house. The bedroom designated for use of children had various items of clothing on the floor- there was also power tools on the floor accessible to the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will clean the inside and outside of the home. Children will not be using the backyard until it is safe for them. Licensee, will submit pictures to LPA of corrections by 10/05/2023.

Based on observation, the licensee did not comply with the section cited above in Poisons were accesible to the children in the living room, bathroom, bedroom and garage which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee, removed poisons during the inspection. Licensee, will review regulation cited and will submit a plan on how they will remain in compliance. Plan will be submitted by 9/25/2023

Based on interview and record review, the licensee did not comply with the section cited above in that Child # 1 was missing sleep logwhich poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with sample sleep log. Per Licensee, she will start documenting sleep as of today and have logs available for department review.

Based on observation, the licensee did not comply with the section cited above in that fire extingusiher was last serviced on 6/25/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will service the fire extinguisher or purchase a new one. Proof of correction will be emailed to LPA by poc due date of 10/5/2023

Based on observation, the licensee did not comply with the section cited above in that LPAs observed various swords and bows and arrows accessible in an unlocked bedroomwhich poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee, locked bedroom during inspection. Licensee, will review regulation cited and will submit a plan on how they will remain in compliance. Plan will be submitted by 9/25/2023
