Based on observation, interview, and record review, the licensee did not comply with the section cited above in LPA observed A2 did not have a fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will discuss with A2 regarding obtaining a fingerprint clearance or to not be present during hours of operation. Proof of fingerprint clearance or written declaration will be submitted to LPA by POC due date.
Based on observation and record review, the licensee did not comply with the section cited above in LPA observed the fire extinguisher was serviced on 11/30/24 which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will service the fire extinguisher and will submit proof to LPA by POC due date.

Resolution: Per Licensee, they will submit a written plan on how they will maintain in compliance. All loose items were removed during the ispection.
