Based on observation, the licensee did not comply with the section cited above in that LPA Additionally, LPA observed different tools and nails accesible in the bakyard that children use- LPA also observed a saw in the living room area which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will submit a written plan on how they will remain in compliance by POC due date. Saw in living room was put away during inspection

Based on(observation, the licensee did not comply with the section cited above in LPA observed acontainer of children's medication on a table located in the living room, Poisons were observed in a locked cabinet under the kitchen. [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will submit a written plan on how they will remain in compliance by POC due date. Medication in living room was put away during inspection- Poisons were locked under key during inspection

Based on observation, the licensee did not comply with the section cited above in that the fire extinguisher expired on 10/5/2022 as indicated on the service tag, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have fire extinguisher service and submit picture of tag to LPA by POC due date of 1/26/2024.

Based on observation, the licensee did not comply with the section cited above in that LPA Lopez observed various tools, aloe vera plant and dog poop accessible to children in the back yard- which poses/posed a potential health, safety or personal rights risk to persons in

Resolution: Per Licensee, they will clean the yard and make area accessible to children safe- pictures will be submitted to LPA by POC due date of 1/26/2024. Per Licensee, they will begin cleaning today and yard will not be accesible until it is ready.
