Based on record review, the licensee did not comply with the section cited above in that assistant did not have proof of current mandated reporter ceritificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have assistant take training and submit copy of completed certificate by POC due date of 4/18/2025

Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed a school age child laying on a cot with her knees bend and folded up to her chest in a toddler cot. During staff interview Licensee, disclosed that they believed that cots were age appropriate for school age children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, They will remove furniture not being used and purchase appropriate furniture for the older children. Per licensee, they will submit pictures of age approriate furniture.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that 4 out 4 infants enrolled are missing documentation of sleep log documenting: Labored breathing, Signs of distress, Infants up to 12 month of age who are sleeping in a position other than on their back. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will begin documenting sleep every 15 minutes and submit copies of sleep logs to LPA by poc due date of 9/22/2023

Based on record review, the licensee did not comply with the section cited above in both Licensee and Assistant do not have proof of current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, he and assistant will take the training and submit copies of certificate to LPA by POC due date of 10/14/2023. Training can be taken on mandatedreporterca.com

Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 infants are missing the LIC 9227 individual sleeping plan which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will provide form to parents and submit completed form to LPA by POC Due date of 10/2/2023
