Based on observation, the licensee did not comply with the section cited above by having unlocked poisons which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immidiately had a neighbor remove poisons from the premises. LPA cleared poc at time of inspection. Licensee will add a pad lock or key lock on storage bins or shed prior to having poisons on the premises.
Based on observation, interview, and record review, the licensee did not comply with the section cited above as both the LPA and LPM did not observe a Mandated Reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of current and completed Mandated Reporter training to the Sacramento Regional Office and Licensing Program Analyst.
