Based on record review, the licensee did not comply with the section cited above in two counts out of two persons which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit current Mandated Reporter certifcates for herself and assistant Mayra Gonzalez to the SDCCRO by the POC date.
Based on LPA observation, the licensee did not comply with the section cited above in that one infant was observed inside a crib which had loose articles, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with PIN 20-24-CCP and a link to NICHQ safe sleep vidoe and quiz. Licensee stated that she will proivde Department with proof of completion of quiz by 10/25/2022.
Based on record review, the licensee did not comply with the section cited above as Licensee's CPR card expired on 03/2022 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will provide proof of CPR certification by 10/25/2022.
