Licensee and S1 had expired Mandated Reporter trainings on file (May 8, 2025).

Resolution: Licensee states she will complete the Mandated Reporter Certificate Training with S1 and submit to the SDCCRO by POC due date of October 10, 2025. Licensee states she will set a reminder on her calendar of the expiration to the Mandated Reporter Training to ensure she maintains in compliance with Title 22 regulations.

Licensee states that S1 does not have a valid First Aid/CPR certificate on file and is occasionally left alone with children.

Resolution: Licensee states S1 will complete and submit a valid First Aid/CPR certification to the SDCCRO by POC due date of October 17, 2025. Licensee states she will set a reminder on her calendar of the expiration to the First Aid CPR certification to ensure she maintains in compliance with Title 22 Regulations.

Resolution: Licensee states she will obtain the LIC 700 and submit a photocoy to the SDCCRO by POC due date of October 10, 2025. Licensee states she will review and ensure that all children have required documentation on file to maintain in compliance with Title 22 Regulations.

Resolution: Licensee states she will obtain signed Individual Infant Sleeping Plans for C1 and C2 and submit a photocopy to the SDCCRO by POC due date of October 10, 2025. Licensee states she will ensure all infants under one years old maintain a signed LIC 9227 in their file.

Resolution: Licensee removed the blankets and stated she will not have blankets in the play yard with a sleeping infant. Licensee stated she will utilize sleep sacks for infants if needed.

Resolution: Licensee removed all items and stated she will ensure a lock or latch is placed on the lower bathroom cabinet. Licensee stated she will ensure the cabinet with chemicals and cleaning solutions in the back yard will be latched. Licensee stated medication and knives will be made inaccessible to children in a latched cabinet.

Resolution: Licensee placed C1 and C2 in play yards during the inspection. Licensee stated she will no longer have infants sleep in a car seat and will immediately place them in a play yard if they fall asleep.

Resolution: LPA provided licensee with a sample sleep log and safe sleep regulations. Licensee stated she will utilize a sleep log and document every 15 minute checks for all sleeping infants. Licensee will submit completed logs to the SDRO by 5/12/23.

Resolution: Licensee stated she will purchase sheets that tightly fit over mattresses and not use blankets in the play yard. Licensee stated she will submit proof to the SDRO by 5/12/23.
