Based on analyst observation, the licensee did not comply with the section cited above as her facility fire extinguisher was displaying as "empty" when inspected by analyst during visit which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will purchase a new extinguisher and send analyst a photo of the purchase receipt by 06/15/23 to complete the correction.
Based on observation and Licensee's statement that she has not taken the Mandated Reporter Training, the licensee did not comply with the section cited above in that she has not completed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 06/15/2023.
