Based on observation, interview, and record review, the director did not comply with the section cited above in that the licensee had an expired CPR card on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will ensure they take the CPR training course and provide proof to the San Diego Regional Care Office (SDRO) by 09/30/2025.
Based on observation and record review, the licensee did not comply with the section cited above in that licensee's mandated reporter certificate is expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will take the mandated reporter training and submit a copy of the certificate to the San Diego Regional Office (SDRO) by 09/30/2025.
Based on record review, the licensee did not comply with the section cited above in Licensee did not renew the Mandated Reporter Training Certificate which expired on 2/24/20, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will take the Mandated Reporter Training and send the department a copy of the updated certicate by email by 4/28/23.
