Based on record review, the licensee did not comply with the section cited above as child #3 was missing multiple documents in his file including a copy of his immunizations which poses/posed a potential health safety or personal rights risk to children in care.
Resolution: Licensee stated that she provided the record packet to the child's parent but she only returned two of them. Licensee states that she will advise the parent of the incomplete record packet and have her obtain the child's current shot records and other missing documents. Licensee states she will then submit copies of the missing records to analyst by 06/26/23 to correct the deficiency.
