Based on statement and observation the licensee did not comply with the section cited above as the carbon monoxide detector was not functioning, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will purchase and install a new carbon monoxide detector and send a video of testing to LPA Poindexter via e-mail by 6/30/25. Email: Gerald.Poindexter@dss.ca.gov

Based on statement, the licensee did not comply with the section cited above in the case of 2 of 2 children, by substituting physical checks with video checks, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will provide a written statement to the epartment that she understands that physical sleep checks are required every 15 minutes, and will send the statement to LPA Poindexter by the POC due date. Email: Gerald.Poindexter@dss.ca.gov

Based on record review licensee and helper did not have current Mandated Reporter certificates on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will provide the Mandated Reporter Training certificates for herself and husband/helper no later than 6/30/25. Mandated Reporter online website address is: mandatedreporterca.com. Email: Gerald.Poindexter@dss.ca.gov

Based on interview and observation, the licensee changed the use of the first (previously on-limits) and second bedrooms (previously off-limits) in the hallway without notifying the department, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The second bedroom to the right in the hallway was added to license per licensee’s request as correction to deficiency. The first bedroom to the right in the hallway is now “off limits.” Deficiency was cleared during inspection.

Based on interview and record review, the licensee did not possess and maintain the safe sleep logs for 2 of 2 infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed the requirement of the Safe Sleep Log with the licensee, who stated she will begin maintaining the logs immediately. Licensee will submit the sleep documentation to LPA by the POC due date and keep updated in the future. Email to Gerald.Poindexter@dss.ca.gov.
