Based on observations and records review there were 5 infants present at the facility which poses/ posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated and records show C1 will turn 24 months in 2 days placing licensee back in ratio. Licensee stated C1 will not return until child is 2 years old.
Based on record review, the licensee did not comply with the section cited above in that she did not have an immunization record on file for Child #1 which poses/posed a potential health, safety or personal rights risk to chldren in care.
Resolution: Licensee states that she will request the immunization records from Child #1's parent today at pick up and will send copies to LPA as proof of correction by 7/1/22.
