Resolution: Mrs. Warshan Satarzada stated that she will help Soghra Hussaini complete the required forms for her child M.H. Licensee will submit copies of child's forms via email no later than 4/24/2025.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee failed to maintain the 15-minute nap log for the infant she has in care. This deficiency poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that the child will return to care on 4/21/2025. She will log the 15-minute nap checks beginning 4/21/2025 and will submit copies of the log to the department via email no later than 4/25/2025.
