TOUSSAINT, HOBESSE FAMILY CHILD CARE

Based on interview with Licensee and record review, the Licensee did not comply with the section cited above in that there were two children (see LIC 811) who did not have immunizations on file. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states that he will have the records completed by 9/15/2023 and email them to licensing.

Based on file review and interview, the Licensee did not comply with the section cited above in that he did not have emergency cards for four of the seven children enrolled (See LIC 811)which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee says he has the emergency contact information in his phone but he will have the emergency card/form filled out by the plan of correction date and email a copy to Licensing for the four children.

Based on file review and interview, the licensee did not comply with the section cited above in that four of the seven children (See LIC 811) enrolled did not have a consent for emergency medical care on file. This is poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee says he will have the emergency medical consent form filled out by the plan of correction date and email a copy to Licensing for the four children.
