Resolution: Licensee will conduct a fire and eathquake drill and send proof of completion to LPA Via email.

Based on observations and record review, the licensee did not comply with the section cited above in 16 out of 16 Children's files has not been completed. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create and complete children's files and send proof of completion to LPA in a PDF format Via email.

Based on observation and record review, the licensee did not comply with the section cited above by not having a current and completed Facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create/complete a facility roster and send proof of completion to LPA Via email.

Based on records review, 4 of 4 children (C1-4) did not have immuniation records on file. This poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: Licensee stated she will submit proof of immunization on record to LPA Ma by POC date.

Based on observation, central heating and A/C unit was accessible which poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: Licensee stated she wil make the heating and A/C unit inaccessible and submit proof to LPA Ma by POC date.

Based on interview and records review, there is not documented emergency and fire drill on record at the facility. This poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: Licensee stated she will complete a fire/emergency drill with the children and submit proof to LPA Ma by the POC date.

Based on interview and records review, licensee did not have an Emergency Disaster Plan on record at the facilty which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete the LIC 610A Emergency Disaster Plan and send proof to LPA Ma by POC date and keep on file at the facility.

Based on observation, alcohol stored on the bottom shelf of a glass cabinet located in the dining room was accessible to children. This poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: This deficiency was corrected during inspection.

Based on records review and interview, Licensee's first aid/CPR certification, completed 2/6/22, was not by EMSA provider. This poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: Licensee stated she will enroll in a pediatric EMSA First Aid/CPR course and provided proof of enrollment to LPA Ma by POC date.

Based on records review, 4 of 4 children (C1-4) did not have signed LIC 995A records on file. This poses a potential hazard to the health and safety of children in care if not corrected.

Resolution: Licensee stated she will have enrolled parents complete, sign LIC 995A and provide proof to LPA Ma by POC date.
