Based on observation, the licensee did not comply with the section cited above. LPA observed overgrown weeds/bush; bar-b-que grill and cleaning tools accessible to children in the backyard which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mr. Nazari stated that he will cut down the weeds/bushes in the backyard no later than 12/14/2023. Mr. Nazari covered the bar-b-que grill and removed the cleaning tools from the yard.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee's helper, Khan Nazari (husband) did not complete the required Mandated Reporter training class which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mr. Nazari stated that he will complete the Mandated Reporter Training per AB1207 and submit a copy of the certificate no later than 12/14/2023.
Based on interview and record review, the licensee did not comply with the section cited above on several children's files. Licensee did not maintain form LIC 700 on 12 out of 17 children's files which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA N. Diaz provided a packet of Children's required forms to Licensee today. Mr. Nazari stated that he will have parents of children complete the required forms and submit copies to the department via email to nancy.diaz@dss.ca.gov no later than 12/14/2023.
