Based on observation, the licensee did not comply with the section cited above in that theres was no carbon monoxide detector in the home to be tested, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will purchase a new carbon monoxide detector. The licensee will send LPA a photo of the installed device and a copy of the store receipt, via email or text by POC due date. Email: Gerald.Poindexter@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above in that ther are tripping hazards in the backyard play area the form of an uneven articificial grass surface and exposed seams, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA stated that she will immediately direct children to use other area of the backyard and block access to the artificial grass area until the area can be repaired. Based on observation, the licensee left Clorox wipes in the unlatched undersink bathroom cabinet and alcoholic beverages in an unlocked/unlatched pantry next to the kitchen, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all chemical items and alcoholic beverages and stored in secure area of the home. This deficiency has been cleared.

Based on interview, licensee had not enabled the crib for the one infant in care and stated that she was using a cushion and not using the crib for infant sleep, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will contruct the child's crib and stated that moving forward, she will utilize the crib for infant annapping. This deficiency has been cleared.

Based on record review and interview, the licensee did not complywith the section cited above, as the licensee did not have personnel files for her adult daughters Silvana Hammo and Steva Hammo, who she identified as helpers, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will create files for both helpers and send proof to the LPA by the POC due date. LPA provided document listing Helper Requirements. Email: Gerald.Poindexter@dss.ca.gov

Based on record review, licensee did not have immunization records in the case of 3 of 3 children in care, children C1, C2, and C3, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will work with parents to provide immunization records for children C1, C2, and C3, and send proof to the LPA by the POC due date. Email: Gerald.Poindexter@dss.ca.gov

Based on record review and interview, the licensee did not comply with the section cited above, as the licensee identified 3 children, C1, C2, C3, for whom she does not have completed and signed paperwork, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will work with parents to provide all paperwork for children C1, C2, and C3, and send proof to the LPA by the POC due date. Email: Gerald.Poindexter@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above, as the licensee did not maintain a current roster. Several children in care, children C1, C2, and C3, were not accounted for on the roster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in that the licensee did not possess and maintain the safe sleep logs for the only infant in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed the requirement of the Safe Sleep Log with the licensee, who stated she will begin maintaining the log immediately. Licensee will submit the sleep documentation to LPA by POC due date and keep updated in the future. Email to Gerald.Poindexter@dss.ca.gov.
