Based on interview and record review, the licensee did not comply with the section cited above. Licensee's husband Nawin Rahimi and helper Rooean Rahimi did not have the required immunization on file - Pertussis, Measle and Influenza which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Rahimi will submit copies of Nawin and Rooean's required immunization to the department no later than 2/20/2024.

Resolution: Mrs. Rahimi stated that her brother-in-law will complete the class online no later than 2/20/24. She will submit a copy of the certificate to the department by 2/20/24.
