Based on records review and interview, the licensee had an expired Mandated Reporter Certificate on file,which poses a potential health, safety or personal rights risk to persons in care.

LPA observed medications in the kitchen drawer and cabinet, cosmetics in the bedroom closet, and a garden rake next to backyard shed, were not made inaccesible to children, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee corrected the deficiencies during the insepection by making all hazardous products inaccessible and placing a latch on the bedroom closet doors.

Based on records review, the last documented emergency drill was 3/18/24, which poses a potential health, safety or personal rights risk to persons in care.

Based on records review, husband and adult son, Ammar and Fabian Hanna, were missing Immunizations records for Measles, Pertussis, and Flu, which poses a potential health, safety or personal rights risk to persons in care.

Based on observartions medicated creams in kitchen and bathroom, many cosmetics products in bathroom and bedrooms, paint and detergent pods in an unlocked shed in the backyard were accessible in the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee corrected the deficiency during the insepection by making all hazardous products inaccessible and placing a lock on the backyard shed.

Based on records review, the last documented emergency drill was 11/20/21, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit prrof of documented emergency drill to the department by 6/9/23.

Based on records review, husband and adult son, Ammar and Fabian Hanna, were missing all helper records (LIC 9108, LIC 9058, negative TB proof, Immunizations, Mandated Reporter Training) after disclosing they help walk the (daycare) children to school sometimes which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of all helper documents for both Ammar and Fabian Hanna to the department by 6/9/23.

Based on records review, Licensee did not have and stated did not know what a children's roster was, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during inspection by completing a children's roster.
