NOORI, SHAMSHAD & ASADULLAH FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above. Licensees failed to maintain the infant's 15-minute nap checks. Licensees only documented the first 15 minute that the child napped. There were no documents available to review to show that the licensees checked on the napping infant every 15 minutes. This deficiency poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided the licensees a sample 15-minute nap log (form). Mr. Noori stated that he will continue to check the infants every 15 minutes during nap and will document them on the form. Mr. Noori will submit copies of infant nap log to the department no later than 5/14/2025.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee has not conducted a fire drill since April 2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mr. Noori stated that he will conduct a fire drill with the 2 groups of children no later than December 8, 2023. He will submit a copy of the fire drill log to the department no later than December 8, 2023.

Based on interview and record review, the licensee did not comply with the section cited above. Mr. Noori and her daughter Bibisara did not have a current Mandated Reporter Training certificate on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mr. Noori stated that they will complete a Mandated Reporter Training and submit copies of certificate to the department no later than December 8, 2023.

Based on interview and record review, the licensee did not comply with the section cited above. Licensees did not maintain current 15-minute infant nap logs on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mr. Noori stated that he will perform and document 15-minute nap checks for children under age 2. Mr. Noori will submit copies of the 15-minute nap checks to the department (for the week of Dec. 4) on 12/8/23.
