Based on observation, interview, and record review, the licensee did not comply with the section cited above in that there was no safe sleep log present in the child's file, and no documentation that the infant is being physically checked on every 15 minutes which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send to LPA by e-mail, a week's worth of copies of completed and filled out safe sleep log for enrolled child, and is due by COB on 04/24/2025.
