Based on interview and review of facility records, licensee has not performed or documented a fire drill every 6 months as required, as the most recent drill was 5/17/25. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a sample copy of Emergency drill log to the licensee, who stated they will conduct a fire drill or earthquake drill no later than 12/11/25. The licensee will email a photo of the documentation to LPA via email by POC date. Email: Gerald.Poindexter@dss.ca.gov

Based on record review licensee's Manadated Reporter Certificate expired 6/1/25 and she did not have a current certificate on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Prior the end of the inspection, the licensee found her updated mandated reporter certificate in her email. This deficiency is cleared.

Based on interview and observation, the licensee allowed children to play in the the off-limits garage, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the children from the off limits garage. Licensee states she understands which areas of her home are off limits and will ensure that off-limits rooms stay inaccessible to children. Ths deficiency is cleared.

Based on record review and interview, the licensee did not comply with the section cited above in the case of 1 of 1 children in care, child C1. Not maintaining immunization records for children poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee, Ms. Momand stated that she will have parents provide the missing immunization records for the children in care and submit copies to LPA Poindexter by 12/11/25. Email to Gerald.Poindexter@dss.ca.gov

Based on interview and record review, the licensee did not comply with the section cited above, as she did not possess and maintain the safe sleep logs for 1 of 1 infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed the requirement of the Safe Sleep Log with the licensee, who stated she will begin maintaining the log immediately. Licensee will submit the sleep documentation to LPA by POC due date and keep updated in the future. Email to Gerald.Poindexter@dss.ca.gov.
