Based on interview and record review, the licensee did not comply with the section cited above as helper Mohamoud Kassim's immunization record does not have proof of MMR vaccine which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that helper Mahamoud Kassim obtains MMR vaccine or antibody test showing immunity to measles and provide proof to LPA no later than close of business on 11/10/21.
Based on record review, the licensee did not comply with the section cited above as two of six children (child #4 and Child #5) do not have immunization record in file. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee will obtain immunization records for child #4 and Child #5 no later than 11/10/21 and will provide records for those children to LPA no later than close of busienss 11/10/21.
