MIKHA, WASAN FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Mikha failed to document the 15-minute nap checks. LPA provided Mrs. Mikha a sample copy of the 15-minute nap check log today. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Mikha stated that she will document the infants' 15-minute nap checks beginning to day for the 2 infants she has in care. Mrs. Mikha will submit copies of completed 15-minute nap checks to the department no later than 6/28/23. Email address is: nancy.diaz@dss.ca.gov

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Mikha's Pediatric CPR/Firs taid certificate expired on 12/2022. This posesa potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Mikha will submit proof of enrollment no later than 6/28/2023. She will submit a copy of the CPR/First aid certificate upon completion.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Mikha failed to obtain completed Sleep plan for the 2 infants she has in care. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above. Mrs. Mikha only has one infant crib. She currently is caring for 2 infants. She stated that one infant sleeps on the sofa. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Mikha stated that she will obtain a play yard by end of business today, 6/21/23. She will submit a photo of the play yard and a copy of the store receipt as proof of correction. Email address: nancy.diaz@dss.ca.gov

Based on LPA's review of facility records and interview with licensee, the licensee did not comply with the section cited above in 9:9 children's records did not contain immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will complete all immunization records for 9:9 children in care and submit proof to LPA via fax or email by POC date.
