Based on observation, 3 of 3 infants were sleeping in infant swings upon entrance. No cribs or play yards were available at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Over the phone, licensee stated she will have individual playpen for each infants under 12 months to sleep in and provide proof to the department via email (patrick.ma@dss.ca.gov) by POC date.
Based on records review, 3 of 3 infants under 12 monts were missing infant sleep plans and 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Over the phone, Licensee stated she will submit completed Infant sleep plans and 15 minute sleep logs for C1 - C3 to the department via email (patrick.ma@dss.ca.gov) by POC date.
