Based on interview, the licensee did not comply with the section cited above. Licensee stated that she has done the fire drills just wasn't aware she had to log them which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will conduct a fire drill and log them and submit proof to LPA by 9/5/25 via email to Annette.Sutherland@dss.ca.gov

Based on observation and interview the licensee did not comply with the section cited above. Licensee and helpers at the home have expired mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and helpers will take mandated reporter training and submit proof to LPA by 9/29/25 via email to Annette.Sutherland@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above. Facility helpers did not have files for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will have helpers sign documents for helper files and have all pending documents and submit proof to LPA by 9/5/25 via email to Annette.Sutherland@dss.ca.gov

Based on record review the licensee did not comply with the section cited above. Children's files were found to be incomplete multiple documents were missing signatures in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will have parents sing all incomplete forms and submit proof to LPA by 9/5/25 via email to Annette.Sutherland@dss.ca.gov

Based on observation and interview, the licensee did not comply with ensuring a criminal record clearance was obtained by adulit resident as the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. In violation for more than 30 days.

Resolution: Licensee states individual will submit fingerprints by 7/2/24. Licensee will ensure all adullts residing, working or have a prominent presence will be fingerprint cleared before their first date in the home or within 30 days of their 18th birthday if living in the home.
