Based upon interviews and review of Name to Face sign in sheets, it was determined that the facility operated preschool classrooms with a ratio of more than 12 children per one teacher for between 2 and 152 minutes on 5/1/23, 5/3/23, 5/4/23, 5/8/23, 5/9/23 and 6/12/23 which posed an immediate health, safety and personal rights risk to children in care.
Based on observation the LIcensee did not comply with the regulation above as toilets had brown rings, debris was caked on fauctes, floors had debris, and windows were smudged with dirt which poses a potential health, safety or personal rights risk to children in care.
