Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: licensee and helper will take mandated reporter training and provide proof of completion to LPA by 7/3/23 and send to Annette.Sutherland@dss.ca.gov
Based on interview, the licensee did not comply with the section cited above. Licnesee has not conducted fire/emergency drills within the last 6 months, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was provided with a fire drill log during todays visit and was advised to conduct it with in the week and submit a copy to LPA by 8/16/22 via email to Annette. Sutherland@dss.ca.gov
Based on observation and interview, the licensee did not comply with the section cited above. Licnesee did not have a Safe Sleep Log, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide a copy of Safe Sleep Log to Licensing via email (Annette.Sutherland@dss.ca.gov) by 8/16/22.
