Based on interview and review of facility records, licensee has not performed or documented a fire drill every 6 months as required. There is no documentation available. This poses a potential safety risk to children in care.

Resolution: POC: LPA provided a sample copy of Emergency drill log to the licensee, who stated they will conduct a fire drill or earthquake drill no later than 12/3/25. The licensee will email a photo of the documentation to LPA via email by POC date. Email: Gerald.Poindexter@dss.ca.gov

Based on inte rview, the licensee did not comply with the section cited above, licensee did not have MMR immunizations for 2 of 4 children, C1 and C2, in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee understands that a complete file must be maintained for each child in care. The Licensee will submit proof of immunizations for children C1 and C2 to the LPA via email at Gerald.Poindexter@dss.ca.gov by 12/3/25.

Based on interview and record review, the licensee did not comply with the section cited above, as she did not possess and maintain the safe sleep logs for 1 of 1 infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed the requirement of the Safe Sleep Log with the licensee, who stated she will begin maintaining the log immediately. Licensee will submit the sleep documentation to LPA by POC due date and keep updated in the future. Email to Gerald.Poindexter@dss.ca.gov.
