Resolution: The licensee states that he will submit sleep documentation for C1, C3 and all infants enrolled to LPA via email by 03/02/23.

Based on observation, the licensee did not comply with the section cited above. The licensee does not maintain an operational smoke detector. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee replaced the battery in the smoke detector at the time of inspection. Citation has been corrected.

Based on record review and licensee statement, the licensee did not comply with the section cited above. The licensee does not maintain a disaster/fire drill log. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that he will conduct and document a disaster/fire drill. The licensee will send LPA a copy of the documentation via email by 3/2/23.

Based on record review, the licensee did not comply with the section cited above. Mandated Reporter certification was not available to review for co-licensee Theresa Christensen. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that he will submit a copy of Theresa Christensen's current mandated reporter certificate to LPA via email by 03/02/23.

Based on record review, the licensee did not comply with the section cited above. Pediatric/Infant/Child cpr/first aid certification was not available to review for co-licensee Theresa Christensen. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that he will submit a copy of Theresa Christensen's current cpr/first aid certificate to LPA via email by 03/02/23.

Based on observation, the licensee did not comply with the section cited above. The neighbors swimming pool is accessible to the children in care. The licensee has a chain link fence, approximatley 4'6" high separating his property from the neighbor's property. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee states that he will reorder the tarp that covers the fence by 2/25/23. The licensee will ensure that the tarp covers the entire chain link fence and that it extends down to the ground so the children cannot climb the fence. The licensee will also ensure that the height of the fence with the tarp is at least 5 feet high. The licensee states that he will send LPA a picture of the completed fence via email.
