Based on investigation interviews, infant who can hold their own bottles should be placed in appropriate seating equipment to feed but are often placed on their backs and provided a bottle if they can support it on their own which, poses/posed a potential health, safety or personal rights risk to children in care.
Based on observation and interviews, infant room sinks does not deliver hot which poses/posed a potential health, safety or personal rights risk to children in care.
Based on investigation interviews, staff are washing infant bottles in classroom sink with dishwashing detergent but not sterilizing as required which poses/posed a potential health, safety or personal rights risk to children in care.
Based on Director statement and interviews, Facility representative did not follow covid guidelines as covid positive result of staff #1 was not reported to the Department verbally within 24 hours or in written form within 7 days, which poses a potential health and safety risk to children in care.
Based on interview and review of relevant documentation, staff had knowledge of an injury occuring to Child #1 while in care, but had not been directly observing the child when the injury occurred, which could have prevented or explained, what occurred. This is a potential risk to the health and safety of children in care.
Based on interview and review of relevant documentation, Staff #1 observed that Child #1 had an injury that needed assessment. This injury was not reported to the child's parent or recorded in the child's record. This is a potential risk to the health and safety of children in care.
Based on observation, LPA observed both diaper changing table vinyl cushions were torn with foam filling exposed which poses/posed a potential health, safety or personal rights risk to children in care.

Based on investigation interviews, partially used prepared infant formula bottles have periodically been saved and refed/offered later in the day. Per CDC, "Throw out any infant formula that is left in the bottle after feeding your baby. The combination of infant formula and your baby’s saliva can cause bacteria to grow." By feeding left over prepared formula this poses/posed a potential health, safety or personal rights risk to children in care.
Based on interviews, Child #1 and possibily other infants in the room, was often fed a bottle in a boppy pillow, with the bottle propped by a blanket or other cloth to hold it in place. This is a potential hazard to children in care.
