Based on observation, the licensee did not comply with the section cited above the garage, stairs, and kitchen were accessible to children in care at arrival which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Correction has been made. Licensee closed the garage, and closed gate to make kitchen inaccessible. .

Based on observation stairs were not barricaded when LPA arrived which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Correction has been made. Licensee has closed gate for stairs in order to make them inaccessible to children.

Based on lack of records, Licensee was unable to provide helpers file for review the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will submit proof helpers S1 file to LPA by 9/26/25 via email Annette.Sutherland@dss.ca.gov

Based on record review the licensee did not comply with the section cited above in, licensee was unable to show proof of S1 immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation , the licensee did not comply with the section cited above in , license is not posted at facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a new copy of facility license and explained to licensee if she needs a new copy to reach out to department for a copy.

Based on record review, the licensee did not comply with the section cited above in, Children's files were found to be incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands that she must have complete files for children in care. Licensee will provide proof of complete files for C1, C2, C3 C4, and submit proof via email to Annette. Sutherland @dss.ca.gov by 9/26/25.

Based on record review, the licensee did not comply with the section cited above, licensee had not updated children that had left or new children that had been added which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee provided updated roster at time of visit. No further action is needed. Licensee understands that she must update LIC 9040 roster when children no longer come to day care and any new children need to be added.

Based on lack of records for infant in care, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands that all children that are present in her home during daycare hours even if child is a helpers child must have a file present in the home.
