BOULES, HADEL FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above. LPA observed a lighting fixture in the dining area that is not anchored appropriately to the ceiling. This condition poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Mrs. Boules stated that she will fix the lighting fixture by end of today. She will submit a photo to show that it was appropriately affixed to the ceiling no later than end of business day of 5/06/2025.

Based on observation, the licensee did not comply with the section cited above. LPA observed an above ground pool that is unfenced and easily accessible to children with about 2 inches of water in the front side yard. This deficiency poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Mrs. Boules stated that she will remove the pool no later than end of business day of 5/6/2025. A photo shall be submitted via email to Nancy.Diaz@dss.ca.gov by end of 5/6/2025.

Based on observation, the licensee did not comply with the section cited above. Mrs. Boules converted her garage. She did not obtain the proper building permit. She also failed to notify the department that she has opened this converted garage to space used by children. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Boules stated that she will not allow children in her converted garage effective today, 10/30/23.

Based on observation, the licensee did not comply with the section cited above. Mrs. Boules failed to notify the department that the garage is now accessible to children. She also failed to notify the department that she moved the outdoor play area next to the garage. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Boules stated that she will move the outside play area back to the front yard. Mrs. Boules was reminded that all equipment (such as the swing or slide) must be attached to the ground. The move will be completed by 11/06/2023.

Based on record review, the licensee did not comply with the section cited above. Licensee's helper Randa Yaqo did not have the required Pertussis immunization on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Boules stated that she will have her helper Randa Yaqo obtain documentation that she has Pertussis immunization no later than 11/06/23.
