Licensee's husband Arturo Nunez and sister Silvia Becerril has not take the online mandated child abuse training.
Resolution: Licensee stated that her husband and sister will complete the online mandated child abuse training and she will submit their certificates to LPA no later than 06/27/2022.
Based on record review, the licensee did not comply with the section cited above in licensee and her husband's measles immunization are not available for review. Licensee's sister Silvia Becerril immunization record for measles and Tdap are not available which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit her and her husband's measles records and her sister's immunization for measles and Tdap to LPA no later than 07/04/22.
