Based on interview and record review, the licensee did not comply with the section cited above, Licensee did not have 6 of 9 children's files for children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of children's files to LPA Annette Sutherland via email to Annette.Sutherland@dss.ca.gov by 2/14/25.

Based on observation, the licensee did not comply with the section cited above. LPA observed infant sleeping in pack n play with a huge stuffed animal which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Correction was made at visit. Licensee removed stuffed animal from pack n play. LPA reviewed safe sleep regulation with Licensee.

Based on interview and record review the licensee did not comply with the section cited above. Licensee has not down safe sleep check for infants since April 2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a sample of safe sleep logs for current infants in care which will include date and 15 min increments with position and check status.

Based on observation, the licensee did not comply with the section cited above in which the licensee had a open face heater in an on limit area which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Correction was corrected at visit. Licensee unplugged the heater and removed it from the on limit area.

Based on observation, the licensee did not comply with the section cited above. Liquid detergent under the kitchen sink was accessible and there was a drawer in the kitchen that consisted of sharp objectives accessible to children. Children's medication was accessible on the counter. Licensee was reminded that all hazardous items need to be latched and locked and out of reach of children which poses an immediate health, safety or personal rights risk to persons in care.. Resolution: Correction was corrected at visit. Licensee removed the liquid detergent from the kitchen cabinet, removed sharp items from kitchen drawer, and moved children medication to an inaccessible location.

Based on observation, the licensee did not comply with the section cited above. Pack n play did not have a fitted shit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LICnesee will purchase a fitted sheet for the pack n play and send proof to LPA via email to Annette. Sutherland@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above. Fitted sheet for pack n play is soiled and has not been washed in many days which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcnesee will make sure she washs the fitted sheet daily or as needed. LICensee will send proof to LPA via email that she has washed the fitted sheet by 2/10/25 to Annette.Sutherland@dss.ca.gov

Based on LPA's record review, the licensee did not comply with the section cited above in 12:12 children's records reviewed were incomplete, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will complete 12:12 children's files and submit an affidavit stating that the files are complete to LPA by POC date via fax or email.

Based onLPA's record review, the licensee did not comply with the section cited above in 3:3 intant's records reviewed did not include the 15 minute check log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will complete the 15 minute check log and submit a copy to LPA via fax or email by POC date.
