ARYOUBI, SADIA FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above. During a tour of the home today, LPA observed that the carbon monoxide present in the home was not functional when tested. This deficiency poses an immediate health, safety or personal rights risk to persons in care.

Resolution: CORRECTED TODAY. Licensee obtained and installed a carbon monoxide detector today.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee's sister Bibi Fatema Aryoubi did not have a fingerprint clearance associated to the facility. Guardian database search conducted today, 2/21/2025 indicated that Bibi's fingerprint process was closed as the department's request for further documentation was not received. This deficiency poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Ms. Aryoubi stated that she will have her sister, Bibi Fatema Aryoubi submit fingerprint via livescan no later than end of business of 2/21/2025.

Based on interview andrecord review, the licensee did not comply with the section cited above. Ms. Aryoubi admitted to not conducting a fire drill since she was licensed in May 2022. The fire drill log that was posted on licensee's wall was blank and did not contain dates of when drills were conducted. This deficiency poses a potential health, safety or personal rights risk to persons in care.

Resolution: Ms. Aryoubi stated that she will conduct a fire drill with the daycare children and submit a copy of the drill log no later than 3/28/2025. She will email a copy of the log to LPA's email: Nancy.diaz@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above. Ms. Aryoubi failed to maintain a current roster of children. This deficiency poses a potential health, safety or personal rights risk to persons in care.

Resolution: CORRECTED TODAY. LPA provided Ms. Aryoubi a blank LIC 9040. She completed form today. LPA obtain a copy of the completed LIC9040 today. Ms. Aryoubi was reminded that she needs to update form everytime a new child enrolls in her day care program.
