Based on record review, the licensee did not comply with the section cited above in 4 out of 4 infants present do not have 15 minute sleep documentation available for review. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit 15 minute sleep documentation for the infants in care (C1-C4) to LPA via email by 10/10/23.

Based on observation, the licensee did not comply with the section cited above. The gate to the swimming pool did not self-latch. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the swimming pool gate repaired so it will self latch when released from all distances. The licensee will send LPA a video of the pool gate closing and latching from difference distance via email by POC due date of 9/29/22. The licensee states that if the gate is unable to be repaired by 9/29/22 she will submit a letter stating how and when the gate will be repaired.

Based on record review, the licensee did not comply with the section cited above. The last documented disaster/fire drill occurred on 11/18/2015. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will conduct and document a disaster/fire drill. The licensee will forward a copy of the documentation to LPA via email by 10/12/22.

Based on record review and licensee statement, the licensee did not comply with the section cited above. The licensee and helper Hortencia Bibo do not have current mandated reporter certification. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit current mandated reporter certificates for herself and helper Hortencia Bibo to LPA via email by 10/12/22.

Based on record review, the licensee did not comply with the section cited above in one out of four children's records reviewed did not contain consent for emergency medical treatment (LIC627) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the parent of C1 complete the consent for emergency medical treatment form, LIC627. The licensee will send LPA a copy of the document via email by 10/12/22.

Based on record review and licensee statement, the licensee did not comply with the section cited above in 4 out of 4 children enrolled do not have signed insurance affidavits on site for review. This poses potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have Insurance Affidavits, LIC282 completed for the four children enrolled (C1-C4). The licensee will forward a copy of the documents to LPA via email by 10/12/22.
