Based on observation, the licensee did not comply with the section cited above. Cleaning supplies are accessible to the children in care in kitchen and bathroom cabinets. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will remove the cleaning supplies in the kitchen cabinet and place a lock on the bathroom cabinet. The licensee states that she will send LPA a photograph/video of the moved and/or locked cleaning supplies via email by 12/15/23.

Based on record review, the licensee did not comply with the section cited above. The licensee has not conducted or documented a disaster/fire drill within the last 6 months. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will conduct and document a disaster/fire drill and send LPA a copy of the documentation via email by 12/15/23.

Based on record review and licensee statement, the licensee did not comply with the section cited above in 5 out of 5 children's files reviewed do not contain an ID and Emergency Information card, LIC700, or Consent for Emergency Medical Treatment, LIC627. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit a copy of the completed and signed ID and Emergency Information Cards (LIC700) and Consent for Emergency Medical Treatment forms (LIC627) for C1-C5 to LPA via email by 12/15/23.

Based on record review, the licensee did not comply with the section cited above. The licensee does not maintain a current roster of the children in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit a copy of her current children's roster to LPA via email by 12/15/23.

Based on record review and licensee statement, the licensee did not comply with the section cited above in 5 out of 5 children's files reviewed do not contain the signed Notification of Parent's Rights, LIC995A. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will submit copies of the signed Notification of Parent's Rights form, LIC995A for C1-C5 to LPA via email by 12/15/23.
